Zayn Malik surprised fans after revealing a tattoo inspired by Drew Barrymore, created during a joint appearance on her talk show. The unexpected exchange, which included matching ink, came shortly before the singer disclosed he had been hospitalized.

The former One Direction member revealed that he now has a tattoo connected to Drew Barrymore. The detail came during an appearance on the Zach Sang Show podcast, in an episode released on April 17, 2026.
Malik explained that the tattoo was done as part of a shared moment with Barrymore, adding a personal layer to the design.
During the podcast, Malik said that he and Barrymore got tattoos together on “live TV.” The comment appears to refer to his appearance on “The Drew Barrymore Show,” which aired on Thursday, April 23, 2026.
The tattoo he received is a simple design located on his forearm. In exchange, Barrymore got a tattoo of Malik’s first name written in Arabic.
The name carries a specific meaning. It translates to “grace” in English, adding significance to the exchange between the two.
The tattoo moment took place before Malik shared news about his health. On April 17, the same day the podcast episode was released, he posted an Instagram Stories photo showing himself in a hospital bed.
He did not provide details about his condition or the reason for his hospitalization. It also remains unclear how long he has been receiving medical care.
Despite the uncertainty surrounding his health, the tattoo exchange with Barrymore had already been filmed before the announcement.
Barrymore has made tattooing a recurring feature on her talk show. Over the years, she has gotten tattoos both on her own and alongside guests.
Among her personal tattoos are designs dedicated to her daughters, Olive and Frankie. These pieces reflect her approach to choosing meaningful and personal ink.
She has also shared the experience with others. In one instance, she and "Today" broadcaster Savannah Guthrie got tattoos together on the show.
Guthrie’s first tattoo on the show read “all my love,” placed near her wrist. The phrase held personal meaning connected to her parents, Charles and Nancy.
Two years later, Guthrie returned for another tattoo session with Barrymore. This time, she chose a “B” with a heart next to it.
Barrymore opted for a different design during that session, selecting a simple tattoo of the word “time.”
Malik’s participation in the tattoo exchange adds him to a list of guests who have taken part in Barrymore’s on-air tradition.
The fact that Barrymore chose to get Malik’s name in Arabic further connects the two through a shared moment that will remain permanent.

While Malik has not shared additional details about the tattoo, the exchange stands out as one of his more unexpected additions.
Some of his other controversial tattoos through the years include those seemingly dedicated to his ex-partner and the mother of his child, Gigi Hadid.
As for his tattoo done during an appearance on “The Drew Barrymore Show,”it reflects a shared experience between the singer and the actress. With both participants choosing meaningful designs, the exchange adds another chapter to Malik’s evolving collection of body art.
