Rosie O’Donnell Reveals Why She Decided to Share Her Face Lift Publicly

The actress opened up about her facelift at the 2026 Tony Awards, revealing her youngest child's blunt reaction, her change of heart, and why she chose to share the news on her own terms.

Rosie O'Donnell has never been one to shy away from the truth — and her latest revelation is no exception.

The talk show host and comedian recently opened up about her decision to go public with her facelift, bringing her signature candor to the red carpet at the 2026 Tony Awards.

"Authenticity is the goal in these days and times," she told E! News at the ceremony this past Sunday. "And I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it."

The decision was deeply personal, but not without some family pushback.

O'Donnell revealed that her youngest child, Clay, was firmly opposed from the start. "My little autistic 13-year-old said to me, 'If you do it, I would not be able to respect you ever again,'" she shared.

That reaction gave her pause. "And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed," she admitted.

Once fully healed, O'Donnell was determined to break the news herself — before any tabloid could beat her to it. She took to social media and Substack, posting before-and-after photos alongside details about the procedure and her doctor.

"I just wanted to say, 'This is what I did, and here's the doctor, and if you want to,'" she explained, though she was quick to add a caveat, "but it's very expensive — it's more expensive than any car I ever bought, but I can't drive around in my face."

As for future procedures, O'Donnell says she's drawing a line. The facelift addressed the only aspects of her appearance that had been bothering her, and she's content to leave it there.

"I'm on Mounjaro over the last three years, I have diabetes too, and I lost over 50 pounds, and that was responsible for a lot of the extra skin that I had around my face," she explained. "And there were two lines that made me look sad."

Those lines apparently became something of a running joke after O'Donnell relocated to Howth, Ireland, in 2025. "In Ireland, people would say, 'Are you upset, darling? What's the matter love?'" she recalled. "And I'm like, 'That's just my face. I am not upset, it's just how I look.'"

When she posted her before-and-after photos late last month, the response was immediate and overwhelmingly positive.

"Very natural and beautiful 🔥," one fan gushed. "It's amazing because it's so natural. She looks beautiful," another echoed. "She looks amazing! When I'm ready, I'm going to her doc 😂 (seriously)," a third quipped.

The admiration kept rolling in.

"Literally looks like a younger version of herself—well done 🙌🏾," one commenter observed. "@rosie you look great! I love that you shared about it too. ❤️," another added. "She looks good. Subtle and tastefully done," a third noted.

In her Substack post, O'Donnell reflected on how dramatically her perspective had shifted. She had long prided herself on being someone who "would never - ever" get cosmetic work, calling the idea "a betrayal. Of feminism. Of aging. Of our team of women worldwide."

Then came the 50-pound weight loss — and with it, a change of heart.

Though Clay's reaction briefly made her reconsider, she ultimately chose what felt right for herself. O'Donnell closed out her post on a quietly triumphant note, framing this chapter of her life as "act 3."

"As I get ready for the last day of school with my youngest - the caboose, here at 64 years old with a new lower face and neck, just happy to be alive. Able to feel and choose and use my voice whenever I feel called to," she wrote. "For the girl I was. The woman I am."
